By signing this document, you waive certain legal rights, including the right to sue.
1. Acknowledgment of Risks
I understand that participation in glass art & resin activities involves inherent risks, including but not limited to:
Cuts or lacerations from handling glass
Exposure to sharp edges and breakable materials
Burns from hot tools
Risks of physical injury due to accidents or equipment malfunction
Inhalation of fumes or fine particles from certain materials
I understand that Fragments Art Studio, LLC provides Personal Protective Equipment (PPE), such as gloves, safety glasses, and aprons, and I agree to use this equipment as instructed. I also acknowledge that despite the use of PPE and adherence to safety protocols, accidents and injuries may still occur.
I acknowledge that I have been informed of these risks and voluntarily choose to participate in these activities.
2. Assumption of Risk
I voluntarily assume full responsibility for any risk of injury, illness, damage, or loss, including death, that may result from my participation in any activity at Fragments Art Studio, LLC whether caused by negligence or otherwise.
3. Release of Liability
In consideration for being allowed to participate, I hereby release, waive, and discharge Fragments Art Studio, LLC, its owners, employees, instructors, volunteers, and affiliates (the “Released Parties”) from any and all liability for any and all claims, demands, actions, or causes of action related to any loss, damage, or injury that may be sustained while participating in studio activities or while on studio premises.
4. Indemnification
I agree to indemnify and hold harmless the Released Parties from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities (including attorney’s fees) arising out of my involvement in studio activities.
5. Medical Treatment
I authorize Fragments Art Studio, LLC to provide first aid and/or seek emergency medical treatment for me if necessary. I understand that I am solely responsible for all costs associated with such treatment.
6. Photography/Video Release (Optional)
I grant permission for Fragments Art Studio, LLC to use photos or videos taken of me during participation for promotional or educational purposes without compensation.
7. Minors
(If participant is under 18) As the parent/legal guardian of the above-named minor, I consent to their participation and agree to all terms of this waiver on their behalf.
